1. A proposed Final Judgment.
2. AS A SUPPORTING DOCUMENT, A cover letter advising the Judge
that this proposed Final Judgment has been approved for submission by the JA for remote

WHEN THE JUDGE REVIEWS THE PROPOSED FINAL JUDGMENT, COVER LETTER AND COURT FILE, SHE WILL EITHER APPROVE YOUR REQUEST OR SEND IT BACK TO YOU WITH INSTRUCTIONS.
Any Final Judgments submitted before your case has been approved for submission by the JA will be rejected.
