("Depository") hereby accepts the designation as the restricted depository for the above-named Ward's assets per the above Court's order dated , 20 . The Depository hereby acknowledges receipt of the following assets ("Assets"):
The Depository certifies that they are qualified to act as restricted depository in accordance with section 69.031, Florida Statutes, and agrees to hold these Assets and all interest, dividends, principal and other indebtedness collected on account thereof, in safekeeping pursuant to Fla. Stat. 69.031, 744.351 (6), and 517.021(6), and to invest, reinvest and disburse the Assets in accordance with the provisions of accordance with chapter 518, Florida Statutes, the order designating it the restricted depository, and any subsequent court order. The Depository shall only release funds from the restricted account upon receipt of a certified copy of a court order directing such release.
This Acceptance of Designation as Depository and Receipt of Assets is executed in the Depository's name by one of its authorized officers on , 20. The original shall be filed with the Court within fifteen (15) days and a copy given to the Guardian of the Ward's Property.
