On the petition of , as the Guardian of the property of the Ward, for authorization to deposit the following assets ("Assets") of the Ward:

, as a depository designated pursuant to Fla. Stat. 69.031, 744.351 (6), and 517.021(6), it is
ADJUDGED:
- The above is designated as the Depository for the Ward's Assets.
- Any person or entity having possession or control over any of the Ward's Assets shall pay and deliver such Assets to the Depository, upon its demand; and the Depository's receipt shall relieve the person or entity from all further responsibility therefor.
- The Depository is authorized and directed to hold these Assets in a fiduciary account for the Ward's benefit and to invest and reinvest the Assets in accordance with the standards set forth in Fla. Stat. 518.11.
- The Depository is authorized to invest these Assets in one or more consolidated funds in which separate fiduciary accounts have undivided interests, provided that separate accounting is maintained for the Assets.
- The Depository shall hold all Assets it receives, and all interest, dividends, principal, and debts collected on account thereof in safekeeping, subject to instructions of the Ward's Guardian of the Property as authorized by court order.
- The Depository shall pay any Florida taxes on the Assets to Florida's Department of Revenue as well as all federal income taxes incurred to the Internal Revenue Service without further order.
- Per Fla. Stat. 69.031(4), the Depository may, from time to time, deduct a reasonable amount for its services and expenses from the Assets it holds without further court order.
- The Depository shall, within 15 days of receiving assets of the ward, file with this Court a receipt for all Assets it received and provide a copy of the receipt to the Ward's Guardian of the Property.
- The Guardian of the Property shall provide a copy of this Order to the Depository within 15 days of the entry of this order, and the Depository shall file with this Court its acceptance or rejection within fifteen 15 days of receipt of a copy of this Order.
