hereby accepts the designation as "Depository" for the above-named Estate per the above Court's order dated .
The Depository certifies that they are qualified to act as restricted depository in accordance with section 69.031, Florida Statutes, and upon receipt of any assets, the Depository will hold, disburse --and if authorized by court order, invest, and reinvest --said assets, the duly collected proceeds therefrom, and all interest, dividends, principal and other indebtedness subsequently collected on account thereof in safekeeping per Section 69.031, Florida Statutes, subject to the instructions of the Estate's Personal Representative as authorized by court order. The Depository, however, will incur no liability in connection with this acceptance until it has filed with the Court its duly executed receipt for the assets.
This Acceptance of Designation as Depository and Receipt of Assets is executed in the Depository's name by one of its authorized officers on . The original shall be filed with the Court within fifteen (15) days and a copy given to the Personal Representative of the Estate.
