The Family Court Self Help Program originated in 1994 through a coalition between the Eleventh Judicial Circuit and the Dade County Bar Association’s Legal Aid Society in response to a need to increase access to the court for Self-Represented Litigants involved in family court matters. The Program’s mission is to provide specialized service to Self-Represented Litigants involved in the family court process by providing the proper forms and instructions to streamline their case, reduce confusion and eliminate delays.
In accordance with limits set by the Supreme Court of Florida, the Self-Help Program can assist Self-Represented Litigants with the following services:
- Initial intake with a front counter specialist
- Online services that provide the Self-Represented Litigants with information on the type of forms packet needed
- Orientation and familiarization of the forms included in the packets so that the self-represented litigant can complete the forms at their convenience.
- Packet Completion Assistance for Self-Represented Litigants needing additional assistance with the completion of the forms. (Optional Service) for a nominal fee.
- Document review service to ensure that forms are inspected for completeness prior to filing with the Clerk of Courts
- Concise instructions on court procedures and what next steps can be expected during this process.
- Connect parties to court partners who can assist with the next stages of the case.
The Self-Help Program’s role is to provide procedural assistance to Self-Represented Litigants for them to present their case to the court and obtain a final hearing. The Self-Help Program does not give legal advice nor guarantee the outcome of the Self-Represented Litigant’s case in court.
The Self-Help Program’s service fee varies from $5 to $125 depending on the Self-Help Program service. The service fee includes the following services: provide procedural assistance, initial review of pleading with a Self-Help Paralegal, notarization of pleadings and documents, copies and postage, packet completion assistance, case follow-up visits, case status information and assist Self-Represented Litigants in obtaining a court hearing.

SHP fees are payable with cash, credit card, or money order. Payments made via money order shall be made to Miami-Dade County. Use this form if you are serving by publication. Disclose the other party’s last known address and thoroughly search all required sources. The court must believe you have made a serious effort to find their location. Follow the instructions provided to meet all diligent search requirements.

Disestablished Paternity
Filing Requirements
This packet should be used by a man who wishes to disestablish paternity or terminate a child support obligation because he is not the biological father of the child(ren). The petition must be filed:
- in the circuit court having jurisdiction over the child support obligation; or
- if the child support was determined administratively and has not been ratified by a court, in the circuit court in which the mother or legal guardian or custodian of the child(ren) resides; or
- if the mother or legal guardian or custodian no longer resides in the state, in the circuit court in the county in which the petitioner resides.
A copy of any judgment or order regarding paternity or child support and a copy of any scientific test results showing that you cannot be the father of the child(ren) must be attached to the petition and filed with the court.

Relocation
Filing Requirements
This packet should be used when you are asking the court to permit the relocation of your principal residence and:
- You plan to relocate your residence more than 50 miles from your principal residence at the time of entry of the last order which established or modified primary residence, custody, visitation, or time-sharing.
- The court has not entered an order granting permission to relocate.
- The relocation will be for a period of 60 consecutive days or more, not including any absence for purposes of vacation, education, or health care for the child(ren).
- Your order regarding custody, primary residence, visitation, time-sharing or parenting plan was entered before October 1, 2009, and the order does not expressly govern the relocation of the child(ren); was entered on or after October 1, 2006; or your case was pending on October 1, 2009.

Temporary Custody
Filing Requirements
This packet should be used by an extended family member to obtain concurrent custody of a child or children pursuant to Chapter 751, Florida Statutes. This form should not be used if you are a parent seeking to establish parental responsibility or time-sharing with a child or children.
An Extended Family Member is:
A relative of a minor child within the third degree by blood or marriage to the parent;
OR
The stepparent of a minor child if the stepparent is currently married to the parent of the
child and is not a party in a pending dissolution, separate maintenance, domestic violence, or
other civil or criminal proceeding in any court of competent jurisdiction involving one or both
of the children’s parents as an adverse party;
OR
An individual who qualifies as “fictive kin.” As defined in section 39.01, Florida
Statutes, fictive kin means a person unrelated by birth, marriage, or adoption who has an
emotionally significant relationship, which possesses the characteristics of a family
relationship, to a child.
